Beyond The Efficiency Index: Finding A Better Way To Reduce Overuse And Increase Efficiency In Physician Care -- Greene et al., 10.1377/hlthaff.27.4.w250 -- Health Affairs: "Current strategies for addressing health care costs stress physician performance measurement and commonly use an efficiency index (EI). During seven years of conducting individual practitioner pay-for-performance (P4P), we found that using EIs hindered our work on reducing overuse of services. This paper offers an alternative approach through the identification of variation in key cost drivers. As proof of concept, we apply this model to hypertension care. We then describe a project that decreased apparent overuse of fiberoptic laryngoscopy among otorhinolaryngologists. Focusing directly on reducing overuse improves cost efficiency without the barriers imposed by EI methodology."

HealthBlawg: PHRs, EHRs, privacy, functionality, and what the future might hold

"Back to the question of regulation: HIPAA seems to have left some gaping holes through which Microsoft's HealthVault and Google Health may pass. Those companies say that their privacy policies are more stringent than HIPAA and Google has said recently that serving ads on Google Health is not in the cards. The problem with relying on these statementsis simply that they are voluntary policies adopted by businesses that may change them over time. Other related parts of the health care information economy are similarly untouched by HIPAA."

In this paper, prepared at the request of the Chairman of the Senate Budget Committee, the Congressional Budget Office (CBO) examines the evidence on the costs and benefits of health information technology, possible barriers to a broader distribution and use of it in hospitals and clinicians’ offices, and possible options for the federal government to promote use of health IT. In keeping with CBO’s mandate to provide objective, impartial analysis, the paper makes no policy recommendations."

Comparison of information technology in general practice in 10 countries[Healthc Q. 2007] - PubMed

Pubmed Abstract:
"A study commissioned by Canada Health Infoway provides a comparative analysis of automation in general practice in 10 countries. The most common clinical application is the automation of medication prescriptions--even if it is not a mandatory requirement as it is in Norway. It is the clinical application that provides one of the biggest benefits to general practitioners as it addresses legibility concerns, can be a significant time saver (particularly for repeat prescriptions) and offers the potential to make use of decision-support capabilities. The transmission of laboratory results is the most common electronic clinical communication application."

Universal Mandatory Health Insurance In The Netherlands: A Model For The United States?

"Policy analysts consider the Netherlands health system a possible model for the United States. Since 2006 all Dutch citizens have to buy standardized individual health insurance coverage from a private insurer. Consumers have an annual choice among insurers, and insurers can selectively contract or integrate with health care providers. Subsidies make health insurance affordable for everyone. A Risk Equalization Fund compensates insurers for enrollees with predictably high medical expenses. The reform is a work in progress. So far the emphasis has been on the health insurance market. The challenge is now to successfully reform the market for the provision of health care."

IBM and HIPAAT Team to Give Patients Control Over Personal Health Information Access

"The IBM-HIPAAT collaboration extends patient-driven privacy to Electronic Medical Records (EMRs), Electronic Health Records (EHRs), Personal Health Records (PHRs) and Health Information Exchanges (HIEs). Combined IBM and HIPAAT technologies allow patients to easily specify who is granted access to their personal health information (PHI), what information can be accessed and when. They enable caregivers to implement and enforce patient consent directives, providing "break the glass" access to PHI and EHR data in emergency-care situations, where appropriate."
